Pain running down the leg can be worrying, and it is often the thing that finally prompts people to seek help. The word sciatica gets thrown around a lot, but not all leg pain is true nerve irritation. Sometimes the piriformis muscle is the real culprit, and the treatment approach is completely different. Knowing the difference saves time, stress, and a lot of guesswork.
What true sciatica feels like
Sciatica happens when the sciatic nerve becomes irritated at the spine. This usually creates sharp or electric pain down the leg, tingling or numbness, pain that worsens with sitting, weakness in the leg or foot, or difficulty straightening the leg fully. It often follows a clear pattern from the lower back into the buttock and down the leg.
What piriformis related pain feels like
The piriformis is a small muscle deep in the buttock. When it becomes tight or overworked, it can irritate the sciatic nerve as it passes underneath. This creates symptoms that mimic sciatica, but the source is muscular, not spinal.
Typical signs include a deep ache in the buttock, pain when sitting for long periods, discomfort when crossing the legs, pain that does not usually go below the knee, and relief when stretching the buttock.
Why people confuse the two
Both conditions involve the sciatic nerve, but the location of the irritation is different. Without a proper assessment, it is easy to assume the worst or treat the wrong area entirely.
What actually works
If the issue is spinal, the focus is on mobility, nerve movement, and stabilising the lower back.
If the issue is muscular, the focus is on releasing the piriformis, improving hip strength, and correcting movement patterns.
Both conditions improve quickly when the right approach is used.
