Shoulder Pain When Lifting Your Arm

Sharp shoulder pain when lifting is common but fixable. LF Therapy helps clients in Esher, Claygate and Oxshott understand the real cause and recover properly.

By Lorenzo Ottanelli, HCPC-registered Physiotherapist

Shoulder pain usually shows up the same way. You lift your arm, reach for something, or try to put on a jacket, and there is a sharp pinch or a deep ache that stops you in your tracks. It is one of the most common issues I see in clinic, particularly among people who spend long hours at a desk or drive regularly.

Most people assume it is a rotator cuff tear or impingement, but the shoulder is more complex than that, and the real cause is often simpler than you think.

Why the shoulder becomes irritated

The shoulder relies on a team of muscles to keep the joint centred as you move. When that team is not working together, the joint shifts slightly out of its ideal position. Even a few millimetres of drift can create irritation.

This usually happens because the rotator cuff is under active, the upper traps are overworking, the shoulder blade is not moving smoothly, the joint is stiff from lack of use, or the chest muscles are tight and pulling everything forward. When these factors combine, the space in the shoulder narrows and the tendons become compressed. That is the pinch you feel.

Why rest does not fix it

Rest reduces irritation, but it does not improve the mechanics. The moment you start using the arm again, the pain returns.

What actually works

A proper shoulder rehabilitation plan focuses on activating the rotator cuff so the joint stays centred, improving shoulder blade control so the movement is smooth, releasing tight muscles that pull the shoulder forward, restoring mobility so the joint can move freely, and building strength so the shoulder can handle daily demands.

When you address the mechanics, the pain settles quickly and stays away.

When to get help

If lifting your arm is painful, if you are avoiding certain movements, or if the shoulder feels weak or unreliable, it is worth getting it assessed. Shoulder issues respond extremely well to early treatment. I see clients in Esher and across Surrey — a proper assessment can help you recover quickly and confidently. Book an assessment →
